HEALTH GUARANTEE
You are required to take your puppy to a
licensed veterinarian for an examination within 48 hours of the final purchase date. At this time should your
veterinarian find the puppy a serious health risk, please contact me within 24 hours. A written diagnosis
explaining and confirming a serious health issue must be submitted from the veterinarian to the breeder. A puppy
may not be determined a serious health risk on account of an injury sustained or illness contracted after the
buyer takes possession of the puppy. The breeder reserves the right to ask for a second corroborating diagnosis
from a licensed veterinarian. The second diagnosis will be performed by a veterinarian of breeder’s choice. If
the puppy is found to be a serious health risk, a next available replacement of equivalent value will be issued
by breeder. All documentation must be supplied and the puppy returned to the breeder as per request prior to a
replacement being issued. If the buyer elects to keep the puppy, buyer assumes all responsibility. In such case,
there will be no replacement, and no additional medical issue will be covered by this guarantee. Once buyer
accepts ownership of the puppy, veterinarian fees and any other service associated with the puppy will be paid
by the buyer.
Your puppy is guaranteed until one year of age
against life threatening congenital disorders. If an issue occurs, written proof explaining and confirming the
life threatening congenital disorder must be submitted to the breeder from a licensed veterinarian or specialist
as the disorder necessitates. Specific diagnostic testing must be performed to confirm the congenital disorder.
The breeder reserves the right to ask for a second corroborating and confirming diagnosis. The second diagnosis
will be performed by a licensed veterinarian or specialist of breeder’s choice. In the unfortunate event that
the puppy becomes deceased prior to confirmation of a life threatening congenital disorder, the breeder reserves
the right to request a necropsy. If a life threatening congenital disorder is confirmed, a next available
replacement of equivalent value will be issued by breeder. All documentation must be supplied and the puppy
returned to the breeder as per request prior to a replacement being issued. If the buyer elects to keep the
puppy, buyer assumes all responsibility. In such case, there will be no replacement, and no additional medical
issue will be covered by this guarantee. Once buyer accepts ownership of the puppy, veterinarian fees and any
other service associated with the puppy will be paid by the buyer.

the Health Folder. Your puppy has received immunization against Canine Distemper, Adenovirus Type 2,
Parainfluenza, and Parvovirus. Your puppy will need to complete the immunization series and a Rabies Vaccine
will be required as specified by your local government. Please review The Bichon Frise Club of America, Inc.
vaccination recommendations, http://bichonhealth.org/HealthInfo/Vaccines.asp , and consult with your
veterinarian regarding immunization necessities and an appropriate vaccine schedule for your Bichon Frise Puppy.
Please be advised certain vaccines can be very dangerous to your puppy. Life threatening side effects and even
fatality can occur. Annual immunization has been linked to autoimmune diseases in many breeds including the
Bichon Frise. It is recommended that a blood titer test be used to determine when immunization is
needed.

understands that the deposit holds a puppy for their purchase and is NOT refundable. Buyer understands the
Breeder must receive the deposit within five days of the contract date unless otherwise notified and agreed
upon.
This contract is legal and binding executed in
the State of Illinois, County of Jo Daviess. It is a non-transferable sales agreement. Should legal action
arise, all legal and court fees are the responsibility of the buyer. If any provision herein is breached, this
contract shall be null and void. The breeder and buyer understand and accept the terms of this contract by their
signatures.
